Intestinal Abscess / Chang Yong
Updated 2026-08-01
INTRODUCTION
English translation pending.
CORE DEFINITION
A pattern in traditional Chinese medicine corresponding to appendicitis. Damp heat and stasis collect in the blind end of the intestine, where qi and blood congest and putrefy into pus. The presentation is severe fixed pain in the right lower abdomen that worsens on pressure, with the right leg held flexed and extension uncomfortable. In the early stage before pus forms, Da Huang Mudan Tang is used to drain heat, break stasis and expel the pus. Once pus has formed, surgery is required.
SCAFFOLDING EFFECT
Reduce cognitive load
- Inspect the Dead End: look for accumulation where flow stops, such as a cul-de-sac or an unused corner - Act Before It Suppurates: clear the blockage early, while it can still be cleared without cutting - Read Guarding as Blockage: treat tenderness on pressure as a sign of retained material rather than of weakness
Anchor fast decisions
Flow through a blind-ended structure is slow by design, so material that enters and does not leave accumulates. Retained damp heat and stasis congest the local blood and qi, and stagnation combined with heat produces putrefaction, which is why the pain is fixed, tender and resistant to pressure rather than diffuse. Clearing the stagnation early removes the substrate before it becomes an abscess that has to be excised.
