CORE DEFINITION
The RACI matrix names, for each task or deliverable, who is Responsible for doing the work, who is Accountable as the single owner of the outcome, who must be Consulted before the decision, and who must be Informed afterward. The core claim is that making these four roles explicit removes the ambiguous boundaries that produce both duplicated work and the vacuum in which everyone assumes someone else is handling it. The key qualification is that accountability must be singular; multiple owners means no one owns the result.
